Required Forms

These forms are required for participation in this event. They may be viewed by clicking on the appropriate wording. All forms are on the Western web site http://www.wrfu.org/. The forms may be downloaded and printed using Adobe Acrobat Reader. If you do not have the Adobe reader installed, go to http://www.adobe.com/ to get the free download and installation of the latest version.

 

Event Roster for Clubs. There are two event roster forms. One is for clubs, and the other is for the All Star Squads.
This form records the names of the squad members you will use for this event. It must be sent to the Western office by close of business the Monday before the event or 23 April 2007. See the Participant Packet for information on the completion of the form. For men's clubs and the Senior Women All Star Squads the Event Roster is limited to 28 players. The Event Roster for Boy's High School teams and Men's Collegiate All Star Squads is open.

A club uses this form to list the players who are scheduled to play in a specific match. See the Participant Packet for information of the completion and use of this form.

 

USA Rugby Liability Waiver Form.  Click here to view and print this form.

USA Rugby requires that all teams have available at all matches signed liability waivers for all members.  Teams will be required to present their team's waivers at check-in.

 

USA Rugby High School Player Eligibility Form. Click here to view this form. This form is also called the Registrar's Form and one must be completed by each school high school players attend. The form is presented at team check in. Players who do NOT appear on this eligibility form will not be allowed to participate on the weekend. The form essentially states that the player is a registered high school student. See USA Rugby eligibility regulations for any exceptions to the rule that a player must be a registered full time high school student.

 


Captain's and Coaches Meeting

A responsible representative from each participating team is required to attend. Uniform colors and styles, match official assignments, Q&As of the assigned officials, any revisions in the schedule, etc., will be covered.

 

Men's Division II Clubs

The meeting is scheduled for 9:30 A.M. on Friday in the pavilion at Gateway Park. This meeting may be replaced by email confirmations.

 

Boy's High School Clubs

The meeting is scheduled for 8:00 A.M. on Saturday in the pavilion at Gateway Park

 

Men's Division III Clubs

The meeting is scheduled for 7:00 AM on Saturday in the pavilion Tent at Gateway Park. This meeting may be replaced by email confirmations.

 

Men's Division I Clubs , Men's Collegiate All Star and Senior Women All Star

There will not be a meeting for these squads.


Club Check-In Schedule

For the finals' weekend check ins are all scheduled for Friday or Saturday morning on the times specified below. Carefully review the Check-In process which is fully described in the Participant Packet. Every team and every player must check in before their first match in the competition. All club check-ins will be conducted at the fields in the pavilion.
